I do not find any
Freedom of Information Act Form that provides a Release of Information.
FOIA is an act that spells out the requirements inorder to release information.

THAT IS WHAT I HAVE BEEN LOOKING FOR. DO YOU KNOW HOW TO GGET THE FORM REQUESTING COPY C-FILE?

To get a copy of your cfile all you need do is go to your closest VARO
and tell them you want a copy of your claims file. They will provide any release you need.
If your VARO is too far away to do this, then write them a note requesting
a copy of your claims file, have your signature notorized and send it to them
return receipt requested. Wait about a week after you get confirmation they received your request then call the 800 # and verify again and ask how long it should take.

That's the point I'm trying to make.
There is no FOIA (Freedom of Information Act) Form.
What they need are Release of Information Forms.

I agree with all, there is no FOIA form..As far as requesting your copies of your C-file...get in line at the St. Pete,Fla. VARO. I have been waiting since 04/12/08. IRIS, does nothing as there is an entirely different dept. that handles those requests and its the FOIA office personnel, im told now(June)..My claim is at the raters, soo i'll leave it alone for now. However, I will pound on the door if the decision is not favorable. Peace, William

When I requested copies of my complete service records the VARO told me to write a letter and specifically write in it I am requesting these under the FREEDOM OF INFORMATION ACT I do hereby request...sign it and send it to them...When I turned the faulty nurse practitioner in to the pt advocate, she had me write a letter to the FOIA director at the hosp and state in the letter I am writing this under the FREEDOM OF INFORMATION ACT...When I requested med records from Iowa City by fax recently I asked them if I needed to use the release of info form and they said nobody uses that here just write a note stating you want copies of your med records under the FREEDOM OF INFORMATION ACT...So these are but a few times I have been told to write this in my letter/requests.
I am not aware of a form, and they all had me write the same statement to retrieve my information. It began with a VARO supervisor who told me that, then VSO too. Guess I have done it 5-6 times already.
If it makes me get what I want, whatever. They do alot of things I don't understand, but do it to get my information or such.
